What it treats

Las Vegas treatment options: the evidence is narrow but real

Testosterone is marketed for energy, mood and focus, and the trial evidence behind Las Vegas prescribing supports a much narrower use than that. The strongest evidence is for hypoactive sexual desire in postmenopausal women. Trials show a measurable improvement in desire and arousal against placebo, and international consensus statements support prescribing for that indication.

Beyond that, the picture thins considerably. Women frequently report better energy, mood, concentration and muscle tone, and clinicians hear it often enough that it is hard to dismiss. The trial evidence for those outcomes is not strong, and we will say so rather than imply otherwise.

If low libido is the main problem, it is also worth checking the things that commonly sit underneath it: vaginal dryness, poor sleep, low mood and relationship factors. How it is used

  1. Estrogen first

    Testosterone is generally added to established hormone therapy rather than started alone. Sorting the estrogen picture first often changes the question.

  2. A small fraction of a male dose

    Typically a compounded cream applied to the thigh, dosed at roughly a tenth of what a man would receive.

  3. Blood levels checked

    Levels are monitored to keep them within the normal female range. Above it, the side effects become both more likely and harder to reverse.

  4. Reviewed at three to six months

    If desire has not improved by six months, continuing is rarely justified and the prescription is stopped.

What to watch

Side effects that do not always reverse

Testosterone side effects divide into the ones that fade after stopping and the ones that may not, and both lists belong in a Las Vegas consultation. At doses kept within the female range, acne and slightly oilier skin are the common complaints and both settle on stopping. Increased facial or body hair is less common and slower to resolve.

Voice deepening and clitoral enlargement are rare at appropriate doses but may be permanent if they occur, which is the main reason levels are monitored rather than assumed. Clinicians therefore prescribe either a compounded preparation or a fraction of a male product. Off-label prescribing is legal and common, but it does mean dosing relies on clinical judgement rather than a licensed regimen.

Yes, testosterone is a controlled substance in the United States. That affects how prescriptions are issued and refilled, and it usually means more frequent review appointments than other hormone therapy requires.
Possibly, but the evidence is much weaker than for sexual desire. Many women report improvement and it would be dishonest to dismiss that, but it would be equally dishonest to promise it. If energy is the main problem, thyroid function, iron and sleep deserve attention first.
Three to six months. If sexual desire has not measurably improved in that time, continuing is not usually justified and we would stop rather than increase the dose.
